Summary

Upgrade dotenv from ^16.5.0 / 16.6.1 to ^18.0.5.

Current / Target

Current: 16.6.1 (wanted within v16)
Latest: 18.0.5

Notes

Major version jump (16 → 18); review changelog for breaking API / default behaviour changes before upgrading.
Deferred from #531 which only applied non-breaking updates.

Acceptance Criteria

dotenv is on v18.x
App still loads env from .env / .env.example as expected
Tests and build pass
